Step 2
Foreign Citizen
Complete the following forms, then bring your immigration documents, i.e.; Passport, Visa, I-94, DS-2019 or I-20, Employment Authorization Card, etc., and Social Security Card to 214 Leigh Hall to complete your payroll signup

- Marketplace Acknowledgment Form
- Foreign National Tax Information Sheet
- Emergency Information Sheet
- I-9 Employment Eligibility
Verification - Disabled Individuals Voluntary Invitation to Self-Identify
- Four-in-One form
- Loyalty Oath (Notarized)
- Intellectual Property Agreement (Notarized)
- Veteran’s Survey
- Race and Ethnicity Information
- Direct Deposit Form
- If you do not have a Social Security Card, please read these instructions.
Step 3
Bring all of your paperwork to 214 LEI Hall, and you are done!
